Geriatric Care Management
Healthy aging requires maintaining daily physical exercise, mental exercise, a regular nutritious meal schedule and socialization.
All Chicagoland Caregivers, now LivHOME, Client Assessments include recording:
- Powers of Attorney the client has assigned
- Advance directives
- Medication list
- Physicians (primary and specialists)
- Record of recent hearing, vision and memory evaluations
- Emergency Lifeline Monitoring Service
- Long-term Care Insurance
- Contact info for family members involved in care decisions
We develop a Care Plan for each of our clients, integrating a daily routine that includes a healthy diet, exercise and mental stimulation. In addition, we engage clients in socialization activities, including accompanying them to local events and programs, when appropriate.
We train the caregivers for each client assignment and actively monitor the caregiver and the care.
Clients who need additional assistance in coordinating and managing all of their senior care issues, such as evaluating the right medical care options and managing their bill payments, may take advantage of our Geriatric Care Management program which includes:
Coordination of care with health professionals, including: Physicians, Psychiatrists, Psychologists, Audiologists, Attorneys, Accountants, Pharmacists, Hospice and Medicare Skilled Home Care.
Communication of care progress to family members and Powers of Attorney: the Care Manager will accompany the client to doctor's visits and other appointments and act as their advocate. A report will be communicated back to all appropriate decision-makers.
Emergency Care Management: the Care Manager will assist out-of-town children and trust officers with on-site management of the client's care. The Care Manager will assist with discharge from the hospital or from a nursing or assisted living community and coordinate all aspects of the care according to the client's wishes.
Long-term Care Insurance Claim Management: the Care Manager will act as the advocate to coordinate all deadlines and necessary documents for processing to insure payment of claims.
Bill payment: the Care Manager will coordinate bill payment and maintain ongoing records.
Household Management: the Care Manager will coordinate necessary handyman repair services, healthy meal planning and grocery shopping and delivery.
